Insurance Quote Forms
Section 1: Medicare Advantage, Supplements & Prescription Drug Plans
- Medicare Advantage: Please fill out this form if you are interested in Medicare Advantage and/or a Prescription Drug plan. I compare all Medicare Advantage and Prescription Drug plans for my clients.
- Medicare Supplement: For those that like predictable outcomes and freedom to use their plan anywhere without network restrictions, preapprovals, and prior authorizations. Quotes will be sent within 24 hours, but often within an hour. I will send a needs assessment form after we discuss Medicare Supplement options. If you decide to work with me, you will need a Prescription Drug plan and I can assist.
- Medicare Prescription: Only fill this form out if you need help with a standalone drug plan that compliments your Medicare Supplement. If you filled out the Medicare Advantage form above, you do not need to do this again.
Section 2: Health Insurance (Under 65) for individuals & families.
A. Individual Health Insurance
- Individual & Family Health Private Underwritten PPO and Marketplace Plans (no preex). Marketplace plans must be in Open Enrollment or you have a special enrollment window. See video below regarding these plans.
- Obamacare Myth Video - Anyone regardless of age, income, sex, ethnicity, etc can get an Obamacare plan during Open Enrollment or Special Election Periods. You may not like it, but EVERYONE can get a policy.
B. Health Sharing Plan (NOT INSURANCE)
- I work with Altrua Healthshare, a non profit healthcare sharing ministry. Plans start at $136 a month. In this brochure are all the plans and pricing for individuals and families. Group plan options are available. If you would like to learn more, please set up a time on my calendar.
C. Short Term Health Insurance
- Short-Term Health (Quote Form) These plans are best suited to fill gaps between jobs.
Short term health plans are not long-term solutions. The timing of a policy expiration date can leave you uninsured.
D. Group Health Insurance
- If you own a business and you're not getting the service you need, please fill out this form and my Group Rep partner will help you out.
Section 3: Dental Insurance
Dental Insurance (quote form) Individual and Family Dental plans for those not receiving benefits fro man aemployer. This form will help me navigate options for you.
